National Kidney Foundation of Michigan now accepting applications for Innovations in Health Care Awards
Every year, the National Kidney Foundation of Michigan (NKFM) and its partners present the Innovations in Health Care Awards. These awards recognize innovations in health care related treatment or prevention programs at the NKFM’s annual Lansing Champion of Hope Tribute Dinner, to be held on April 30, 2013. The NKFM is currently accepting applications for the 2013 Innovations in Health Care Awards.
This year, the NKFM is recognizing innovations in the following categories:
· Innovative Projects in Implementing Healthy Behaviors: For projects that promote making healthy choices, specifically related to the four health behaviors of Governor Snyder’s 4 x 4 Plan—maintaining a healthy diet, getting regular exercise, getting an annual physical exam, and quitting smoking.
· Innovative Projects Related to Knowing Your Numbers: For projects that promote knowing the four health indicators of Governor Snyder’s 4 x 4 Plan—body mass index, blood pressure, cholesterol, and blood sugar.
· Innovative Projects in Kidney Disease, Dialysis, & Transplantation: For projects demonstrating innovations in the treatment of kidney disease, dialysis, and kidney transplantation.
All programs stimulating change, creating awareness, or improving the quality of care should consider applying. Applicants from the health care sector, community at large, educational systems, community health programs, and health plans are encouraged to apply.
The top 12 applicants will be invited to present a poster at the Lansing Champion of Hope Tribute Dinner on April 30, 2013 at the Kellogg Hotel and Conference Center, where 3 applicants will be awarded. Winners will receive $1,000 along with a plaque for advancing their program.
